Reablement Review Officer
Salary: £32,076 - £33,024
Hours: 34 hours per week
Location: Cumbria House, Carlisle
Contract: Temporary (12 months)
About this role:
This post will be temporary for 12 months working within the Reablement and support at home teams.
The role will support Reablement discharge to assess patients returning home from hospital and community hospital settings developing a strength-based plan to maximise and maintain independence.
To assess, plan and closely review community Reablement packages to maximise independence working alongside adult social care, ICC, district hospitals and the wider support at home teams to evidence any need for long term support.

DBS
This role is exempt from the Rehabilitation of Offenders Act (ROA) 1974 and requires an enhanced DBS check with a Adults Barred List check.
Please note this role involves working in regulated activity with adults. It is an offence for people who are barred from working in regulated activity to apply for roles that require them to work unsupervised with adults.
