Enhanced Transition Worker
An exciting opportunity has arisen for an experienced, enthusiastic, and motivated practitioner to join our friendly and supportive team based in Carlisle. The Learning Disability, Autism and Transition Team works with adults and also with young people from the age of 14 years, who are transitioning into adulthood. As an Enhanced Transition Worker, you will be working specifically with young people to assess their needs and support them as they move from children’s services to adult services.
The successful applicant will be an experienced practitioner who is able to work autonomously and has the necessary skills to develop effective approaches in the management of complex casework and promote strength-based practice.
You will supervise others within the team and in doing so, support their continuing professional development, as well as working collaboratively with internal and external partners.

This role is exempt from the Rehabilitation of Offenders Act (ROA) 1974 and requires an enhanced DBS check with a Adults Barred List check.
Please note this role involves working in regulated activity with adults. It is an offence for people who are barred from working in regulated activity to apply for roles that require them to work unsupervised with adults.
