The advertised salary includes a supplement (JWC) to the base grade of the post. JWC supplements are conditional on the physical demands and /or working conditions of the role.

About the service
The support at home service is part of People and provides both domiciliary and reablement services as part of the wider provider services team.
Support at Home is a 365 day a year 7 day a week 24-hour service. This will include weekend and Bank Holiday rotas.
The domiciliary service operates during the day and at night. The team of support workers provide a service to individuals based on a strength-based support plan which will identify tasks and assistance required to enable activities of daily living to be undertaken within individuals’ own home.
Reablement is a short-term intervention which operates during the day; its purpose is to maximise independence for individuals, this is done through the creation of a support plan which identifies goals for individuals to work towards.
About the role
This is an exciting opportunity to work as part of the support at home team providing short term support to individuals with dignity and respect. Within your role you will be completing assessments of individuals needs after a period of illness or a change in personal circumstances to ensure they access the correct service.
This will enable you to develop a strength based, person centred support plan, developed with the individual focussing upon the outcomes they would like to achieve.
To review and move people through the reablement pathway in a timely and efficient manner, maximising the use of internal and external resources, including occupational therapy support. Evidencing robustly the need for any ongoing support required as part of a long-term package of care.
You will be working alongside trusted assessors in the hospitals who identify individuals who have Reablement potential to support their discharge from the hospital setting.
You will also work alongside social workers who have identified individuals in the community who may require a period of reablement in order to support them to remain at home.
To ensure the individual reaches their full potential you will need to take a holistic approach to an individual’s support, working alongside other professionals, including GP’s, District Nurses, Physio’s and the third sector to ensure that the best possible outcomes are achieved for the individual.
This role will be mainly based in South Lakes and Eden, although there may be a service need to support in Furness on occasion.
Due to the nature if this role there will be a need to respond to service users who will not always be accessible via public transport; therefore, there is a requirement to travel independently via a motorised vehicle.

DBS
This role is exempt from the Rehabilitation of Offenders Act (ROA) 1974 and requires an enhanced DBS check with a Adults Barred List check.
Please note this role involves working in regulated activity. It is an offence for people who are barred from working in regulated activity to apply for roles that require them to work unsupervised with adults.
