Salary: 31 346 - 32 234 (pro rata)
The advertised salary includes a supplement (JWC) to the base grade of the post. JWC supplements are conditional on the physical demands and /or working conditions of the role.

The RoleLong Term Assessment and Review ServiceThis team will support people, aged 18+, who are living in the community and have been identified as having longer term care and support needs. The service will work across health, social care and third sector organisations with a primary focus upon ensuring that all support plans:
